SSW Patient Navigator

Job Description:

Gather psychosocial information to participate in the development and implementation of a plan of care that provides extraordinary care for the patient while being sensitive to costs and resources. Work collaboratively with patients, patients' family/significant other, healthcare providers, insurers, community resources, and other involved parties. The SSW will execute these duties under direct clinical supervision.

Position Details:

Shift: 8am -4:30pm on Tuesday's and Thursday's. No weekend or holiday requirement but they can work on a holiday if it falls on the day of a scheduled shift, but would not be required to.

Department: Bbehavioral health and an access center

Minimum Qualifications:

• State of Utah SSW license
• Three years of experience working with Behavioral Health Patients in a behavioral health care setting.
• Excellent interpersonal and communication skills.
• Experience adapting quickly when the need arises.
• Knowledge of available behavioral health community resources.

Preferred Qualifications:

• Post high school education in a related field of study.
• Experience with discharge planning for behavioral health patients.

The hourly range for this position is listed below. Actual hourly rate dependent upon experience. 

$29.43 - $45.39
