How you’ll make an impact in this role
Comprehensive Psychosocial Assessments: Interview residents and families to evaluate social, emotional, and financial needs, ensuring all case activity is meticulously documented to support individualized care planning.
Interdisciplinary Care Collaboration: Partner closely with clinical managers, families, and external healthcare agencies to ensure full alignment and regulatory compliance with the resident's long-term treatment goals.
Community Resource Navigation: Assist residents and families in securing essential community assistance, healthcare benefits, and eldercare resources through targeted referrals and advocacy.
Resident Safety & Advocacy: Proactively identify environmental safety hazards or emergency situations, initiating immediate corrective action to maintain a secure, high-quality living environment for all residents.
- Maintain strict CMS compliance by accurately completing federally mandated MDS assessments (including Section Q) within required timelines to support resident psychosocial well-being and safeguard facility funding.
What minimum requirements you’ll need
Licensure / Certification / Registration:
- Master Social Worker, preferred.
Education:
- High school diploma/GED with 2 years of experience, or Associate's degree, or Bachelor's degree required.
Work Experience:
- 3 years of experience preferred.
