Nurse Navigator - Full time, Days - Cardiology - Atlantic Health Chilton Medical Center

• Exceptional communication, organization and customer service skills.
• The Nurse Navigator is responsible for enhancing the communication amongst the patient/family, the interdisciplinary team and all interdepartmental needs.
• Assists in the development of strategies to reduce LOS and re-admissions. 
• Part of the cardiovascular team which monitors LOS and re-admission data for CHFAMI, COPD, and pneumonia.
• Monitors care process and develops approach to improve care efficiency and patient outcomes. 
• Performs other related duties as assigned.

Schedule:

• Full time, Days
• 8am - 4pm

Required Education:
 

• Bachelor's/Master's in Nursing or related field

 

License/Certification

• Current NJ (New Jersey) Registered Nurse License required
• BLS required

 

Relevant Experience

• 3 years of cardiology nursing
• Quality background preferred
