Position Summary

  • Location: Work From Home – Flexible, Travel Required: 70% (Barry, Van Buren, Kalamazoo, Calhoun, Branch, St Joseph, Cass, and Berrien Counties)

  • Schedule: Standard business hours Monday-Friday 8:00am-5:00pm EST

  • No evenings, weekends, or major holidays

  • Eligible to work 4 day/10-hour schedule available after 6 months

Our Mission

Community Health Workers (CHW) serve the community as a bridge between the member (community) and the health care system (providers) through outreach and education. By providing these services, CHW's help Aetna members attain and maintain better health outcomes, improve relationships with healthcare providers and help both parties become more acculturated with each other.

This position will work under the direction of a designated professional.

Key Responsibilities

  • Requires flexibility to work in situations in which the working conditions are unknown, as environments and situation of patient homes may vary.

  • Ability to communicate medical information to health care professionals and care coordinators over the telephone.

  • Knowledge of formal and informal community agencies and resources.

  • Working knowledge of multi-system outreach programs related to health care delivery, clinical education, and health-related services.

  • Strong interpersonal and communication skills and the ability to work effectively with a wide range of constituencies in a diverse community.

  • Occasional weekend hours may be required.

  • Requires the ability to travel to multiple office locations and home locations.

  • Able to influence medical care providers as needed to adjust interventions based on member’s cultural, ethnic, or religious preferences.

  • Ability to work collaboratively in a team and manage multiple priorities, utilizes effective time management skills, and exercise sound judgment.

  • Exceptional organizational and interpersonal skills, with attention to detail required.

  • Ability to effectively provide support care to socially and medically complex patients in a variety of non-traditional settings.

Remote Work Expectations

  • This is a remote role and candidates must have a dedicated workspace free of interruptions.

  • Dependents must have separate care arrangements during work hours, as continuous care responsibilities during shift times are not permitted.

Required Qualifications

  • Must reside in Barry, Van Buren, Kalamazoo, Calhoun, Branch, St Joseph, Cass, and Berrien Counties, or the surrounding service area.

  • Must be willing and able to travel within a 90-mile radius up to 70% of the time.

  • Must possess reliable transportation and maintain the ability to travel throughout the assigned territory.

  • 1-3 years of relevant experience required, preferably in community outreach, member engagement, patient support, care coordination, or conducting home visits.

  • Experience working with diverse urban and/or rural populations, with a strong understanding of local community resources, support networks, and service organizations.

Preferred Qualifications

  • Experience working in community/outpatient setting.

  • Community Health Worker Certification or equivalent training and experience.

  • Preferred skills: Mental Health First Aid, Trauma Informed Care, HIPAA, C.L.A.S. Standards, Motivational Interviewing, and specialized training in medical terminology and practices for designated health conditions.

  • Meet all requirements set forth by local state i.e. first aid, CPR, TB screen, etc.

  • Experience using a computer including the use of Excel and Electronic Health Records.

Education

  • High School Diploma or G.E.D

Pay Range

The typical pay range for this role is:

$18.50 - $35.29

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls.  The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.  This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
