Medicaid Eligibility Specialist

Location - St Joseph PACE

Shift - 1st Shift

Hours - 8-4:30 Monday - Friday

In person - No Remote

We are seeking a compassionate and detail-oriented Medicaid Eligibility Specialist to support older adults and their caregivers through the Medicaid application and eligibility process. In this role, you will serve as a trusted resource, helping participants navigate Medicaid requirements while ensuring timely and accurate submission of applications, re-determinations, and supporting documentation.

Key Responsibilities

  • Guide participants and families through Medicaid applications, approvals, and re-determinations.
  • Conduct home visits to complete applications and gather required information.
  • Collaborate with county agencies, participants, caregivers, and internal teams to ensure eligibility requirements are met.
  • Educate families on Medicaid rules and regulations, including Long-Term Care Medicaid, trusts, and spousal impoverishment provisions.
  • Monitor and track eligibility status, maintaining accurate records and documentation.
  • Complete HMO disenrollment paperwork and communicate progress to enrollment and leadership teams.
  • Maintain strict confidentiality and compliance with all federal, state, and organizational requirements.

Qualifications

  • High school diploma or equivalent required.
  • Comprehensive knowledge of Medicaid programs, eligibility requirements, and benefits.
  • Understanding of intake practices and geriatric assessment principles.
  • Knowledge of community resources available to seniors and their families.
  • Minimum of one (1) year of experience working with frail, elderly, or aging populations.
  • Strong communication, organizational, and customer service skills.
  • Ability to work independently while building collaborative relationships with participants, families, and team members.
