Remote Behavioral Health Case Manager (Licensed)
100% Remote | Monday–Friday | No Nights, Weekends, or Holidays

What You'll Do
- Conduct comprehensive behavioral health and psychosocial assessments
- Develop and manage individualized care plans
- Coordinate care with providers and community resources
- Support members through crisis intervention, referrals, and ongoing follow-up
- Use motivational interviewing and coaching techniques to drive positive outcomes
- Identify and address quality-of-care concerns
- Maintain accurate, compliant clinical documentation
WHAT YOU BRING - REQUIRED
- Master’s degree in social work, Counseling, Psychology, or a related field
- Active, unrestricted clinical license (LCSW, LSW, LISW, LCPC, LPC, LP, LMFT, LMHC, LMSW, or equivalent)
- 3+ years of post-master’s direct clinical experience
- 3+ years of experience treating mental health and substance use disorders
- 3+ years of case management experience
- Experience with crisis intervention and motivational interviewing
- Strong technical skills and ability to thrive in a fast-paced, telephonic environment
Preferred Qualifications
- Managed care, care management, or utilization review experience
- Discharge planning and care coordination experience

The typical pay range for this role is:
$54,095.00 - $116,760.00This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
