Social Worker LCSW (Per Diem) - Palliative Care
This is a per diem/on call inpatient position supporting patients in Santa Rosa Hospital but will be supporting Healdsburg and Petaluma Valley as needed.
Provides psychosocial assessment and delivery of counseling, social work and emotional support services to terminally-ill patients, and/or palliative services to patients and their families. Works as a team member to identify problems and assist patient/family members in exploring realistic options, decision making and problem solving.

Required Qualifications:
- Master's Degree - Social Work, Psychology or Counseling. Graduate from an accredited school preferred or,
- Ph.D. -Social Work, Psychology or Counseling. Graduate from an accredited school preferred.
- Coursework/Training - Training in family counseling and case management.
- California Clinical Social Worker License upon hire.
- 1 year - Experience in a health care setting or related field.
