Mental Health Counselor - Partial Hospitalization Program - MSM - Full Time/Days
Under general supervision, serves as the primary intake and admissions coordinator for Adult Partial Hospitalization Program (PHP) and Intensive Outpatient Program (IOP) services, with approximately 0.5 FTE dedicated to each program. Reviews and coordinates referrals from inpatient units, emergency departments, ambulatory programs, community providers, and external referral sources. Perform clinical screenings, admission coordination, and treatment team collaboration to ensure appropriate level-of-care placement and successful program engagement. Provides milieu support and serves as a backup group facilitator when needed. Functions within the scope of practice authorized by New York State licensure and Mount Sinai Health System policies and procedures. Candidates must possess and maintain an active New York State LMHC or LMFT license in good standing.
- Serves as the primary intake coordinator for both PHP and IOP programs.
- Reviews incoming referrals from inpatient, ED, ambulatory, and community referral sources.
- Outreach to referral sources on changes in programming and/or answer questions on referral process
- Conducts EMR reviews to obtain clinical history, risk factors, and treatment information.
- Conducts clinical screenings for prospective patients
- Coordinates with treatment teams regarding admissions and level-of-care recommendations.
- Coordinates admission schedule and patient onboarding.
- Serves as a point person during milieu hours for patient support and brief interventions.
- Provides crisis intervention and de-escalation within scope of practice.
- Serves as backup group facilitator
- Documents screenings, assessments, and interventions in Epic/EMR.
- Performs other related duties.
Education Requirements
Master?s Degree in Mental Health Counseling, Marriage and Family Therapy, or related behavioral health field required.
Experience Requirements
1-year supervised experience conducting therapy for specific patient population.
Licensing and Certification Requirements (if applicable)
Current New York State Licensed Mental Health Counselor (LMHC) or Licensed Marriage and Family Therapist (LMFT) license required.
