Catholic Health Home Care is looking for passionate, dedicated LPNs to provide comprehensive care to patients at home.
Team 5: Bohemia, Brentwood, Fire Island, Great River, Islandia, Islip , Islip Terrace, Lake Ronkonkoma, Oakdale, & West Sayville.
- 5x 8 hour shifts (8:30AM to 4:30PM) - Rotating Weekends

Job Details
Our Home Care LPN’s:
- Case manage, assess, plan, develop and implement a comprehensive home health care plan for their patients.
- Measure vital signs, Administer IVs, change dressings, clean wounds as needed
- Provides skilled nursing care, preventative rehabilitative procedures, and prescribed treatments with a variety of patient populations within various potentially complex home situations.
- Work closely with a supportive patient care team including physicians, therapy and social services, pastoral care, home health aides and others for your assigned caseload.
- Provide patient and family education for disease management and health promotion s directed by physician’s plan of treatment.
Minimum Qualifications:
- New York State Licensed Practical Nurse (LPN) license
- Minimum of one year of recent hands on experience
- Basic Life Support Certification
Posted Salary Range
USD $33.00 - USD $40.00 /Yr.This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job, and it does not include the value of benefits.
