LPN LVN - Home Health

Schedule- Full Time/Days

Area Coverage: Dallas, Northwest Salem, Corvallis and surrounding areas

The LPN provides quality care to patients in the nursing center and their homes under the direction of a Nurse Case Manager or Clinical Supervisor following the established nursing process and procedures, and in accordance with the Nurse Practice Act. Performs all duties in compliance with all applicable State and Federal regulations and in compliance with all regulatory bodies. LPN's working in Home Health require 25% to 35% of time on daily travel.

Required Qualifications:

  • Education to meet certification, license or registration requirement.
  • Upon hire: Oregon Licensed Practical Nurse 
  • Within 30 days of hire: National Provider BLS - American Heart Association
  •  Driving is a necessary as part of this role. Caregivers are required to comply with all state laws and requirements for driving. Caregivers will be expected to provide proof of driver license and auto insurance upon request. See policy for additional information. 
  • 1 year Experience in Acute Care, Home Health, public health nursing, or patient care experience in a skilled nursing facility or medical clinic.
