The Admissions Liaison is responsible for collecting and inputting information gathered via detailed patient chart review, communication with patient, communication with patient family and communication with medical team in order to present the data to the Medical admissions team, in order to determine the appropriateness of admission to the acute rehab unit and whether the patient meets acute rehab unit admissions criteria. The Admission liaison is responsible for completing the pre-admissions screen forms, electronically forwarding to the physician, verifying benefits, getting insurance authorization when needed, and insuring CMS compliance throughout the acute rehab admission process This position must collaborate and interact in a positive manner with PPS coordinator, case managers, physicians, discharge planners and other potential referral sources. They are responsible to build and maintain positive relationships with referral sources and families with the goal of increasing the number of ARU referral sources, ARU referrals, and ultimately the ARU census. The Liaison will work with the program director to develop and implement a marketing plan.

Minimum Education:
High School or equivalent
Bachelor’s Degree OR

Additional Education Requirements
Five years of related experience in lieu of Bachelor's Degree.

Minimum Experience:
2 years Must have minimum of 2 years’ experience in inpatient acute rehabilitation

Required Certifications:
Licensed Vocational Nurse - LVN (CA DCA) Valid California LVN License.


Preferred Certifications:
Basic Life Support (BLS) Healthcare Provider from American Heart Association

Pay Transparency

The hourly salary range for this position is $34.32 - 56.19.
