Full-Time Hospice On-Call Registered Nurse (RN )- After-Hours Care in the Capital Region and Saratoga county area - Evenings/Overnights

The Community Hospice is seeking a compassionate and dedicated Full-Time Registered Nurse (RN) to join our After-Hours Hospice Care Team. In this rewarding role, you will provide critical nursing care and support to hospice patients and families in their homes and nursing facilities throughout the Capital Region and Saratoga area during evening and overnight hours.

Schedule - Full-time evening or overnight shifts. Every other weekend commitment required.

What you will do:

  • As an After-Hours Hospice RN, you will respond to patient and family needs during evenings and overnight hours providing skilled nursing assessments, symptom management, and supportive care in homes and nursing facilities.
  • Collaborate closely with the hospice care team to ensure continuity of care and promote comfort, dignity, and quality of life.

Responsibilities:

  • Provide after-hours nursing visits to hospice patients in home and facility settings
  • Assess and manage symptoms, addressing urgent patient and family needs.
  • Provide education, guidance, and emotional support to patients and caregivers.
  • Coordinate care with the interdisciplinary hospice team.
  • Communicate changes in patient condition and ensure appropriate follow-up.
  • Maintain accurate and timely clinical documentation.
  • Work with interdisciplinary team.

What you will need: 

  • Valid NYS RN license required.
  • 1-2 years previous RN experience required, homecare and/or Hospice experience preferred.
  • Strong assessment, communication, and critical thinking skills.
  • Ability to learn/use an electronic medical record
  • Clean Driver's License and reliable vehicle
  • Hospice experience is preferred but not required. We welcome RNs who are passionate about providing compassionate care and are eager to learn. Our comprehensive orientation and training programs will help you build the skills necessary to succeed in hospice nursing.

Pay Range for non-per diem: $38.00-$50.18 Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location
