Assess patient health status, identifying actual and potential health concernsDevelop and implement individualized care plans using evidence-based practicesEvaluate patient responses and adjust care plans to optimize outcomesEngage patients and families as active participants in care planning and deliveryCoordinate care across interdisciplinary teams to ensure continuity and qualityDelegate and supervise care provided by Licensed Practical Nurses (LPNs) and Unlicensed Assistive Personnel (UAP)Promote a positive unit culture and contribute to a collaborative work environmentUphold standards of patient advocacy, safety, and clinical excellence
Qualifications
