Clover Health is looking for exceptional, mission-driven physicians to join our field-based In-Home Care team. In this role, you will care for Medicare Advantage members with complex medical, functional, and social needs — many of whom are older, medically fragile, and at high risk for avoidable complications.

This is not transactional care. This is relationship-centered, outcomes-driven medicine.

As a Field-Based Staff Physician, you will be accountable for the health outcomes of a defined panel of members. You will meet patients where they are, understand the realities of their home environment, identify risks early, and help coordinate the right care at the right time. You will have the autonomy to practice thoughtfully, the support of an interdisciplinary team, and the technology to act with greater insight and precision.

Many of the patients we serve are navigating multiple chronic conditions, recent hospitalizations, fragmented care, limited resources, and the challenges that come with aging. For these members, the home is often where the most important clinical information lives.

In this role, you will have the opportunity to see the full picture: the medications on the kitchen table, the caregiver dynamics, the fall risks, the barriers to follow-up, and the early warning signs that may never surface in a clinic visit.

Your work will directly influence whether members recover safely, remain stable, avoid unnecessary hospitalizations, close care gaps, and feel supported by a care team that genuinely knows them.

What you’ll do

As a Field-Based Staff Physician on Clover’s In-Home Care team, you will:

  • Provide high-quality, patient-facing care in members’ homes across New Jersey
  • Manage a panel of medically complex Medicare Advantage members and take full accountability for clinical outcomes
  • Conduct comprehensive in-home assessments that evaluate medical, functional, behavioral, and social needs
  • Lead post-discharge and transitional care visits to support safe recovery and reduce avoidable readmissions
  • Identify changes in clinical status early and intervene proactively
  • Manage chronic conditions, medication concerns, care gaps, and acute clinical needs within the home-based setting
  • Build trusted relationships with members, caregivers, and families
  • Collaborate closely with nurses, care managers, pharmacists, advanced practice clinicians, primary care providers, specialists, and community partners
  • Use Clover Assistant and other clinical technology tools to surface risks, guide evidence-based decisions, and deliver more proactive care
  • Communicate clearly and efficiently with internal and external care teams
  • Document accurately and thoughtfully to support continuity, quality, and accountability
  • Help advance a care model built around value, outcomes, and human connection

What you bring

  • MD or DO degree
  • Board certification in Internal Medicine, Family Medicine, Emergency Medicine, or a related primary care field
  • Geriatrics experience or fellowship training preferred
  • Active New Jersey medical license, DEA registration, and CDS registration
  • Strong clinical judgment and experience managing chronic conditions in older adults
  • Comfort caring for medically complex, frail, and socially vulnerable patients
  • Ability to work independently in a field-based environment
  • Strong documentation, communication, and care coordination skills
  • Curiosity and proficiency with clinical technology, including the ability to use tools such as Clover Assistant to make more informed decisions and identify care opportunities
  • Experience in home-based care, value-based care, population health, complex care, or Medicare Advantage is a plus
