Overview
The Palliative Care Resource Liaison serves as a hospital-based advocate and educational resource for patients, families and medical care team members to facilitate best practices for end-of-life and chronic care services.
Responsibilities
The Liaison:
- Collaborates with medical team members to identify needs and advocates for patients and families needing palliative care or hospice services.
- Supports discussions about patient and family concerns and goals of care to include health care surrogacy, advanced care planning, and symptom management.
Qualifications
Required Education: Bachelor's in Nursing OR Masters in Social Work or related field.
Necessary Skills:
1. Critical thinking skills
2. Effective communicator with difficult conversations about goals of care
3. End of life or chronic care services
4. Ability to develop positive relationships
Preferred Licensure/ Certifications: Licensed Clinical Social Worker Advance Care Planning Certified Hospice and Palliative Nurse (CHPN)
Required Experience: 1-2 years working with hospice and/or palliative care patients
Preferred Experience: 5+ years working with hospice and/or palliative care patients
