The Outreach Program Coordinator is responsible for assisting in the development and dissemination of inclusive promotional and educational materials tailored to the needs of the grant's target communities, East Harlem and the South Bronx. They will also provide direct, empathetic guidance to patients seeking care at the Long COVID clinic and work collaboratively with the clinical care team. Build and maintain trusted relationships with community members, community-based organizations (CBOs), and the Community Advisory Board.

  • Coordinates with Long COVID clinician(s), other providers and social work, as needed. 
  • Weekly clinic schedule maintenance such as scheduling, insurance verification, referrals management, etc as needed
  • Document all grant-required reporting initiatives with patients to quantify for both QI initiatives and internal documentation.
  • Assists in the creation and dissemination of promotional and educational materials. 
  • Liaises between Clinic Leadership and community based organizations serving as a resource for community based education, screening, symposiums, educational sessions etc. 
  • Present information effectively to diverse audiences.
  • Some weekends required.
  • Other duties as assigned.
  • Bachelor’s degree in Public Health, Social Sciences etc. or equivalent combination of education and experience.
  • One (1) year work-related experience.
