Job Summary and Responsibilities

As our  Rehab and Admission Discharge Social Worker, you will play a vital role in obtaining payor authorizations for rehabilitation admission.

 

Every day, you will perform social work assessments, interventions as needed, and planning for patients discharge needs. You will manage multiple priorities to create ethical, compassionate care plans.

 

To be successful in this role, you will must have excellent understanding of rehab admission criteria, both pre-admission, admission and discharge and insurance payor.

 

 

  • Creative, compassionate problem solving.
  • Effective collaboration with payors.
  • Timely documentation for admission authorization review.
  • Chart Review.

  • Knowledge and experience obtaining prompt authorization from payors prior to patient admission to the Rehabilitation Center.
  • Experience performing psychosocial assessments to identify discharge barriers.
  • Coordinating safe discharge plans, including equipment and home care needs.
