The Field Care Manager Nurse 2 assesses and evaluates member needs to support optimal health and wellness outcomes. In this role, you will partner with members, families, and care teams to develop and implement individualized care plans, coordinate services, and connect members to resources that address both clinical and social needs.This is a field-based and telephonic position, where in-person versus telephonic engagement is guided by member needs, contractual requirements, and regulatory expectations.
The Field Care Manager Nurse 2 performs advanced nursing activities including:
Conduct comprehensive assessments to identify member needs, risks, and barriers to care
Develop and manage member-centered care plans with measurable goals and outcomes
Coordinate care across providers, community support systems
Monitor progress and adjusting interventions to ensure outcomes
Address social determinants of health, including housing, transportation, and access to care
Utilize evidence-based care pathways to guide clinical decision-making
Engage members using coaching and motivational interviewing techniques
Field-Based Responsibilities:
You will require travel to complete in-person visits with members in different environments to support holistic assessment and care coordination.
Visits may occur in:
Members' homes
Group homes or community living settings
Hospitals or skilled nursing facilities
Homeless shelters or community locations
Correctional facilities (e.g., prisons or jails) as part of reentry and transition planning
You will also conduct telephonic outreach and follow-up as part of ongoing care management.
Important: Field visits—including those in nontraditional settings such as correctional facilities—are an expected and essential function of this role to ensure continuity of care and successful community reintegration.

Required Qualifications
Reside in Eastern Kentucky (strongly preferred)
Active Registered Nurse (RN) license
Bachelor's degree in nursing or healthcare/human services related field.
2+ years of experience working as a case manager or care manager
Experience working with adult populations
Knowledge of community health services, social services, and available resources
Proficiency in Microsoft Office (Word, Outlook, Excel) and electronic medical records
Preferred Qualifications
3+ years of care coordination or community-based experience
Managed care experience
Experience with health coaching and wellness promotion
Case Management Certification (CCM or similar)
Motivational Interviewing training
Experience working with Pediatric populations and their families
Bilingual (English/Spanish)
Additional Information
· Schedule/Time Zone: Monday through Friday, 8:00 AM – 5:00 PM Central Time Zone.
· Work Location: Eastern Kentucky (strongly preferred)
· Work Style: Field
· Travel Requirements: Up to 25% within a 120-mile radius to complete field visits.
TB Statement: This role is considered patient facing and is part of Humana's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.
Driving Statement: This role is part of Humana's driver safety program and therefore requires an individual to have a valid state driver's license and are expected to maintain personal vehicle liability insurance. Individual must carry vehicle insurance in accordance with their residing state minimum required limits, or $25,000 bodily injury per person/$25,000 bodily injury per event /$10,000 for property damage or whichever is higher.

At minimum, a download speed of 25 Mbps and an upload speed of 10 Mbps is required; wireless, wired cable or DSL connection is suggested. In certain roles, the minimum recommended internet speed required by Humana may not be sufficient for business needs. Humana reserves the right to require associates to upgrade their internet service if necessary.

Work from a dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information.

Travel: While this is a remote position, occasional travel to Humana's offices for training or meetings may be required.

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.
