Performs physical assessments as necessary and interpret clinical findingsCertifies level of care determinationsWorks closely with physicians and other health care professionals. Consults with team members and participates in team care management activitiesDevelops care plans, implements and monitors services, consults with other team members, completes case recording and reporting.
