Job Summary and Responsibilities
As our Care Manager RN, you will be a dynamic leader, shaping clinical excellence and empowering your frontline nursing team to deliver unparalleled patient care.Every day, you will inspire and guide your department, translating organizational goals into action that elevates patient experiences, strengthens community ties, and ensures consistent delivery of quality care. Your exceptional communication will transform every interaction into a powerful opportunity to elevate understanding and propel our nursing services to new heights.Conducts in-depth assessments/reassessments covering medical, health, and rehabilitation concerns. Performs physical assessments as necessary and interpret clinical findingsCertifies level of care determinationsWorks closely with physicians and other health care professionals. Consults with team members and participates in team care management activitiesDevelops care plans, implements and monitors services, consults with other team members, completes case recording and reporting. Maintains case loadParticipates with CCT providing assessments, feedback and planning for patientsAssists with the development of organizational programs to create new services
Job Requirements
Required:Three (3) years experience two (2) of which are in public healthAssociate of Science degree in nursingValid current California license to practice nursing Preferred:Geriatrics experience Bachelors degree in nursing
