Patient Navigator, LVN - Beverly Hills IM Endo Clinic - Full Time 8 Hour Days (Non-Exempt)(Non-Union)
The Patient Navigator is a Licensed Vocational Nurse with recognized leadership abilities and sound clinical skills who will actively assist the physician in the day to day activities and care of the patient. This role will also include arrangement of the details of patient care in the unit's service line. The LVN Patient Navigator takes a leading role in activities in the unit, patient rounding and care, identifying and assessing patient needs, scheduling and working with the Physicians, RN coordinator, employees, and preceptors in the orientation and training of new employees. Working in close collaboration with the RNs and M.D.s, preparing, teaching and guiding patients through the continuum of care. Executes an expertise patient care, Support of Systems, Research, Education, and Professional Leadership. He/she works collaboratively with the nursing staff, medical staff, educators, managers, and all members of the health care team to accomplish the responsibilities outlined below.
Departmental Specific Summary

This role will also include an arrangement of detailed patient care in the primary care service line. Takes accountability for the patients' beginning-to-end experience regardless of their needs. Assists patients in eliminating barriers to care by referring them to internal and community resources and/or appropriate staff members. Assists/coordinates patient appointments and follow-ups. The Patient Navigator LVN assists the multidisciplinary team in managing the care of the patient through all phases (In-take/Evaluation/Listed) by acting as a liaison between the clinic and referring physicians. Serves as resource and liaison to external vendors by building relationships with relevant industry vendors that will provide value to the patient base. This includes relaying important health education/training to faculty and staff
Essential Duties
Initiates and coordinates patient teaching regarding care and follow – up under the direction of the Providers and clinic leadership.
Reviews all medications and conducts medication reconciliation
Communicates with the medical team and treating/attending physician
Under the supervision of the Manager and attending Physician, oversee patient flow through the continuum of care including test scheduling, scheduling, assisting through the admitting process
Coordinates consults with physicians and appropriate services such as social service, nutritional services, case management, transportation, radiology, lab etc.
Under the direction of the Manager views, follows and evaluates laboratory results.
As directed, is responsible for patient contact and communication liaison between patient and health care team, and community partners
Under the direction of Manager and/or Physician, is involved in Patient and family education – initial and follow up – disease process, health care system navigation and referral management, pre and post op care and expectations etc.
Serves as resource and liaison to external vendors by building relationships with relevant industry vendors that will provide value to the patient base. This includes relaying important health education/training to faculty and staff
Performs other duties as assigned.
Along with the RN Coordinator coordinates and facilitates the monthly Bladder Cancer Support Group with Social Services
Under the direction of the RN Coordinator Views, follows and evaluates laboratory results.
Required Qualifications:
Req High school or equivalent
Req Specialized/technical training Graduate of an accredited Vocational Nursing Program
Req 2 years Experience in acute care or ambulatory care setting as Patient Navigator or LVN.
Req Excellent communication skills.
Req Ability to speak, read and write English proficiently.
Req Excellent time management, Organizational, Interpersonal , and Research skills.
Req Ability to negotiate.
Req Demonstrate excellent customer service behavior.
Req Able to function independently and as a member of the team.
Req Must be well-versed in Word processing, Microsoft Outlook, PowerPoint, Excel, electronic health records as well as utilization.
Req Ability to effectively work with physicians, staff, and patients.
Req Knowledge of medical terminology.
Preferred Qualifications:
Pref 1 - 2 years Experience in case management
Required Licenses/Certifications:
Req Licensed Vocational Nurse - LVN (CA DCA)
Req Basic Life Support (BLS) Healthcare Provider from American Heart Association
Req Fire Life Safety Training (LA City) If no card upon hire, one must be obtained within 30 days of hire and maintained by renewal before expiration date. (Required within LA City only)

The hourly rate range for this position is $34.32 - $56.19.
