Health Information Management Representative

Schedule: Monday-Friday, day shift

The Health Information Representative is responsible for creation and review of Plans of Care to ensure CMS program compliance is met. This position handles technical reviews, audits and maintenance of Home & Community Care patient records.  This role works collaboratively with the clinician and provider to ensure certifications, orders and clinical documentation throughout the patients' care have been completed and medical record issues that impact the revenue cycle process are resolved in a timely manner.  This position ensures documents are scanned, indexed and retained in a manner that safeguards the integrity of the medical record.

Required Qualifications: 

  • 2 years experience in a team setting which may include participation in a healthcare-related college program.
  • Six (6) months of the experience must be in a clerical or general office setting; clinical setting preferred.

Preferred Qualifications:

  • Coursework/Training: Health Information Management, computer science, business, or related health care studies
  • Coursework/Training: Medical Terminology
  • Upon hire: OASIS Certification 
  • 6 months experience with an Electronic Medical Record (EMR) system.
