Job Summary and Responsibilities As our Strategic Partnership and Care Navigation Coordinator, you will orchestrate and improve care coordination plans between CommonSpirit Health, strategic partners, and specialist services, prioritizing humankindness and patient-centered care.Every day you will analyze performance trends of strategic partnerships, facilitate timely communication paths using patient and provider data, implement process systems with nursing and allied health staff, and develop standard work for medical record transmission and insurance authorization.
