We serve patients and their loved ones in Lake of the Woods County and the surrounding area.  If you are committed to social justice, health equity, and prepared to deliver care in new, innovative ways, you belong with us.  Job Summary and Responsibilities As a CNA at CHI LakeWood Health,  you will help our patients by providing nursing care to our Long Term Care (LTC) residents under the direction of the physician or registered nurse. Every day you will work with our patients to evaluate their needs and provide physical or psychosocial care.  To be successful in this role, you must have a genuine compassion for your patients’ well-being and understand the role you play in helping our our patients feel at ease. You also have the patience to explain the process, answer questions and update documentation.  To thrive in this role you will be organized and prioritize accordingly. You will demonstrate the ability to work independently; have strong written and verbal skills with the ability to engage others. Your leadership skills will  allow you to handle conflicts resulting in a positive outcome.  Contributes to the assessment of the health status of patientsPerform administrative requirements, such as completing necessary forms, charts, reports, etc., and submit these as may be requiredCommunicate patient information with care teamsAssist patients/families with meals or changing bed linensReports any changes in a patient's condition to the Charge NurseMust have ability to work effectively and collaboratively in a diverse environmentWe are offering a $3,750 Sign-On Bonus to new hires who meet the eligibility requirements.
