The Senior Post Acute Transition Specialist functions as a highly specialized strategist, exercising significant autonomy to spearhead post-acute care initiatives for a flagship Enterprise Practice and its affiliated health system. This role serves as an executive-level Account Manager, architecting deep-seated partnerships with health system leadership and orchestrating the care continuum across the local healthcare ecosystem. The specialist is responsible for synthesizing sophisticated data insights into high-impact operational workflows, delivering quantifiable improvements in wellness metrics and driving enhanced ACO financial performance through rigorous strategic alignment. The specialist is responsible for synthesizing sophisticated data insights into high-impact operational workflows, delivering quantifiable improvements in wellness metrics and driving enhanced ACO financial performance through rigorous strategic alignment.

Primary Duties

    1. Enterprise Account Management & Strategy:

      1. Act as the principal post-acute authority and centralized liaison for the flagship Enterprise Practice.

      2. Serve as an executive-level Account Manager, fostering deep-seated partnerships with practice leadership and hospital-based population health navigators across the care continuum.

      3. Exercise significant autonomy in spearheading all post-acute initiatives for the Enterprise Practice, ensuring rigorous alignment with Aledade’s overarching strategic objectives.

    2. Data-Driven Program Leadership:

      1. Develop and present sophisticated collateral that synthesizes claims data, clinical insights, and performance analytics into high-impact operational workflows.

      2. Deliver quantifiable improvements in patient wellness metrics, including length of stay (LOS) and readmission rates, to drive enhanced ACO financial performance.

      3. Orchestrate strategic quarterly Town Halls to align diverse stakeholders—including SNFs, hospitals, and practice leads—on collaborative problem-solving and shared goals.

    3. Advanced Clinical & Workflow Execution:

      1. Utilize advanced clinical expertise and understanding of social determinants of health to champion optimized transition of care strategies for complex patient populations.

      2. Independently architect and refine specialized protocols for SNF patient management, establishing a standard of excellence for Enterprise Practice operations.

      3. Perform all core responsibilities of the Post Acute Transition Specialist with an elevated degree of strategic insight, professional accountability, and operational independence.

Minimum Qualifications

    • Bachelor's degree in a clinical or healthcare-related discipline, or a verified equivalent in professional experience, is required.

    • Minimum of 5+ years of clinical experience within a complex healthcare environment. .

    • Minimum of 6+ years of professional experience operating within a value-based care landscape, such as an ACO, MCO, or large-scale health system.

    • Demonstrated competency in exercising high levels of autonomy, spearheading complex strategic projects, and managing executive-level stakeholder relationships.

    • Advanced proficiency in architecting and delivering data-driven collateral to drive change management and influence strategic decision-making.

    • Direct experience collaborating with post-acute care providers and health system personnel to optimize the care continuum.

Preferred KSA’s

    • Proven track record in architecting and operationalizing strategic programs within complex healthcare environments.

    • Superior public speaking and facilitation capabilities, with the ability to engage and influence diverse executive-level stakeholders.

    • Possession of a verified and active nursing credential as a Registered Nurse (RN), Licensed Practical Nurse (LPN), or recognition as an International Medical Graduate (IMG), is required.

    • Previous experience in high-level Account Management, driving strategic alignment and fostering deep-seated professional partnerships.

Physical Requirements

    • Sitting for prolonged periods of time. Extensive use of computers and keyboard. Occasional walking and lifting may be required.

    • Travel up to 25%
