Care & Community Relationship Manager
Role responsibilities
Conduct in-home client assessments and build care plans while maintaining relationships with clients and families. Engage with the community to build referral relationships and partner with the caregiving team to ensure quality care.
Requirements
Candidates should be warm, personable, and able to connect with clients easily. A clinical background is a plus, but not required; strong judgment and the ability to operate with limited structure are essential.
