Job Summary and Responsibilities

As our Family Services Coordinator, you will conduct comprehensive family assessments, develop individualized family plans, and provide crucial in-home and on-site case management services to enhance family stability and support early childhood education.Every day you will manage a caseload of 35-40 families, offering family support, parenting education, and social services including referrals, advocacy, and services brokering, all while collaborating with a flexible team to ensure high customer satisfaction.To be successful in this role, you will demonstrate strong assessment and case management skills, a deep understanding of family support needs, excellent communication and advocacy abilities, and a commitment to empowering families and ensuring optimal outcomes for young children and pregnant women.

  • Complete a comprehensive assessment of each family with respect to health care; education; mental health and family relationships; children's development; employment and training, child care; housing and legal issues; parent-child interactions; and the actual physical care provided by parents.
  • Develop an individualized family plan that incorporates family strengths and needs and promotes self-sufficiency in each of the above areas.
  • Support caregivers through home visits and center-based contacts to develop organized and safe environments; identify children's developmental strengths, temperamental characteristics, and communication styles; and learn about normal development, age appropriate toys and play activities, nutrition, and routine health care need.
  • Facilitate and provide leadership for group socialization experiences, parent trainings, and parent conferences.
  • On an ongoing basis, identify emerging mental health, health, parenting, developmental, and environmental issues and provide individual and family referrals, advocacy, support, and anticipatory guidance as needed.
  • Collaborate and act as liaison with community agency representatives at various organizational levels to facilitate and coordinate provision of needed services, including, as necessary, convening joint case conferences with staff from other involved agencies and making joint home visits when interagency or interdisciplinary collaboration is indicated.

Job Requirements

Required

  • Bachelors degree in child development,social work, psychology, human development, or a related field.
  • Experience working with low-income families and families from diverse cultural and ethnic backgrounds.
  • Skill in organizing and maintaining accurate records.
  • Driver's License: CA
  • Auto Insurance, upon hire and
  • Basic Life Support - CPR, within 90 Days
  • First Aid, within 90 Days
  • Fire Safety, within 90 Days
