Discover a meaningful role where you’ll engage with health plan members primarily online, helping to close gaps in care.Signify Health, part of CVS Health®, is hiring full-time, board-certified nurse practitioners or physician assistants to conduct Video Visits for Medicare Advantage, Medicaid, and certain commercial plan members. During our Video Visits, you’ll conduct an approximately 30-minute visit, including a medication and medical history review, allowing time to educate members about their health and answer their questions.In this role, you will:Conduct online “Video Visits” with health plan members, working in a private space with adequate internet connectionBe part of our efforts to visit millions of people nationwide with the goal of providing connections to the right care for people’s unique needsProvide 40 hours of availability weekly, consistently providing 320+ capacity units per monthExecute a strong visit completion rate, consistently pacing 220+ completed visits per monthMaintain training, affiliation, and compliance with Video Visit requirements (regular training required)Other role expectations and requirements:Be trained and credentialed for all health plan clients and lines of business in primary coverage state(s)Consistently engage with regular manager, team and clinical meetingsBe accountable to Signify Health operational, clinical, and member experience key performance indicators and metricsMaintain active state licenses in coverage areas (FNP, AGNP certification preferred for NPs)Maintain board certificationExperience with Signify Health In-Home Health Evaluations, Video Visits, in-home visits, telehealth visit delivery, or a similar format (preferred)Able to obtain and maintain multi-state licenses in coverage area, as requiredAdditional Required Skills, Abilities and Working Environment:Clinicians will be dedicated to providing Video Visits (“telehealth”) services onlyMust meet minimum requirements for remote visit delivery, including broadband connectivity with video and audio enabled, a quiet setting with a neutral background from which to conduct visits, and the ability to uphold health plan member privacy per CVS Health guidelines.While performing the duties of the job, the employee is regularly required to interact with health plan members in a remote manner, sit, write, operate the computer and phone, speak intelligibly, and hear member responsesSpecific vision abilities include the ability to view and read a computer screen, iPad, and other electronic devicesEffective verbal, written, and electronic communication skillsAbility to work without direct supervision and practice autonomouslyProficiency with information management and technologyDiscover the stability of becoming a full-time SignifierFull-time employees qualify for our comprehensive benefits package, including:The security of a base salary plus incentive structure (based on the number of in-home visits completed)Malpractice insurance coverage (during Signify Health-related activities)Anticipated Weekly Hours40Time TypeFull timePay RangeThe typical pay range for this role is:$95,738.00 - $206,206.00This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls.  The actual base salary offer will...
