The Field Care Manager, Behavioral Health 2 assesses and evaluates member's needs. We do this to achieve and maintain optimal wellness state. The care manager guides members/families toward resources appropriate for the care and wellbeing of members, facilitating interaction with these resources. The Field Care Manager, Behavioral Health 2 work assignments are varied and frequently require interpretation and independent determination of the appropriate courses of action.You will report to Manager, Care Management Behavioral Health.
The Field Care Manager, Behavioral Health 2 employs a variety of strategies and techniques to manage a member's physical, environmental and psycho-social health issues. Resolve barriers that hinder care. Ensure patient is progressing towards desired outcomes by managing patient care through assessments and evaluations. May create member care plans. Understand department, segment, and organizational strategy and operating goals, including their linkages to related areas.
Please note that field work is a mandatory requirement for this position. You will travel up to 25% of the time. Applicants should reside within the state of South Carolina and preference will be given to those in the Upstate or Midlands area.

Required Qualifications
- An active, unrestricted SC Licensed Masters Social Worker (LMSW), OR with an active, unrestricted compact Registered Nurse (RN) license.
- 2+ years or more of experience working as a behavioral health professional
- Must reside and perform work in the state of South Carolina and willing to travel to see members up to 25% of the time
- Must have a separate room with a locked door that can be used as a home office to ensure continuous privacy while you work
- Experience with behavioral change, health promotion, coaching and wellness
- Proficient in using electronic information systems and Microsoft Office tools (Word, Outlook, Excel) to document, communicate, and collaborate effectively with internal partners and members.
Preferred Qualifications
- Experience supporting patients telephonically
- Experience working in Community Mental Health or as part of a crisis response team or Assertive Community Treatment (ACT) team, and substance use disorder treatment
- Experience working with both children and adults
- Ideal location Upstate or Midlands
Additional Information
Workstyle: Combination remote work at home and onsite/home member visits
Field - this role will travel up to 25% of the time to visit member homes or facilities in the area.
Hours: Must be able to work a 40 hour work week, Monday through Friday 8:00 AM to 5:00 PM, over-time may be requested to meet business needs and requires approval.

TB Statement
This role is considered patient facing and is part of Humana’s Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.
Driving Statement
This role is part of Humana's driver safety program and therefore requires an individual to have a valid state driver's license and are expected to maintain personal vehicle liability insurance. Individuals must carry vehicle insurance in accordance with their residing state minimum required limits, or $25,000 bodily injury per person/$25,000 bodily injury per event /$10,000 for property damage or whichever is higher.
Mileage reimbursement is provided for work-related travel. Eligible mileage includes:
- Travel from your home to your first work location of the day.
- Travel between client or assignment locations during the workday.
- Travel from your final work location back to your home.
Travel: While this is a remote position, occasional travel to Humana's offices for training or meetings may be required.

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.
