Accountabilities:
- Lead and oversee hospice quality improvement activities using continuous quality improvement principles and methodologies.
- Develop, implement, and evaluate clinical education programs, training curriculums, and professional development initiatives.
- Ensure compliance with Medicare Conditions of Participation (CoPs), accreditation standards, and applicable healthcare regulations.
- Manage and support education and quality teams, ensuring effective execution and sustainability of learning programs.
- Collaborate with clinical leadership and Human Resources to coordinate onboarding, orientation, and ongoing staff development.
- Establish processes to monitor and improve patient care quality, safety, risk management, infection control, documentation accuracy, and customer satisfaction.
- Develop and maintain documentation improvement initiatives, including task forces and audit processes.
- Monitor quality metrics, analyze performance trends, and provide recommendations to leadership for continuous improvement.
- Oversee audit management processes, including chart reviews and corrective action planning.
- Support regulatory readiness by staying current with hospice standards, Medicare requirements, and industry best practices.
- Manage key performance indicators (KPIs) and report progress to executive leadership.
- Collaborate with external vendors and internal teams to optimize educational resources and operational effectiveness.
- Contribute to strategic program development and initiatives that support organizational growth.
- Bachelor’s degree in Nursing required; Master’s degree preferred.
- Active and current Registered Nurse (RN) license.
- 7+ years of clinical experience in hospice or home health environments preferred.
- 5+ years of supervisory or leadership experience managing teams.
- Experience with state and CMS survey processes, accreditation requirements, and regulatory compliance.
- Strong understanding of Medicare regulations, Hospice Conditions of Participation (CoPs), QAPI, and ACHC accreditation standards.
- Demonstrated experience developing and improving organizational quality metrics.
- Knowledge of medical terminology, documentation standards, and basic ICD-10 coding.
- Experience with hospice documentation systems such as Homecare Homebase (HCHB) preferred.
- Strong understanding of adult learning principles and experience delivering training programs.
- Proficiency with Microsoft Office applications, including Outlook, Word, Excel, and PowerPoint.
- Excellent organizational skills with the ability to manage multiple priorities and deadlines.
