Job Summary and Responsibilities
As a Navigator RN, you will provide expert nursing guidance and support to patients as they navigate complex healthcare journeys.Every day you will assess patient needs, educate on treatment options, coordinate care across multiple providers, and facilitate access to resources, acting as a central point of contact.To be successful, you will demonstrate strong clinical knowledge, exceptional communication and problem-solving skills, and a compassionate, patient-centered approach to ensure seamless and informed care.
- Provide oversight of the disease registry database
- Provide oversight or conducts pre-visit chart review of patients
- Works with patients and families on Self-Management Support
- Provides Coordination of Care across the care continuum
- Involvement in QI activities
Job Requirements
Required
- Graduate of accredited school of nursing approved by the State Board of Nursing and Associate’s degree
- Registered Nurse: MN
- Basic Life Support - CPR
Preferred
- Minimum of three years of recent nursing work experience, preferably including experience in conducting nursing education
- Clinic/Physician office experience and background in diabetes eduation.
