Accountabilities:
- Review member medical histories, lab results, medications, and clinical documentation to assess eligibility for enrollment in care programs.
- Apply established intake protocols, decision trees, and clinical assessment frameworks to ensure accurate and consistent enrollment decisions.
- Coordinate across multidisciplinary care teams, including nurses, providers, health coaches, and operations staff, to support a seamless member onboarding experience.
- Escalate cases to providers when additional clinical evaluation is required and ensure appropriate program placement decisions.
- Utilize digital tools, dashboards, and tracking systems to manage intake workflows, documentation, and communication efficiently.
- Provide timely, accurate clinical documentation and maintain strict adherence to confidentiality, quality standards, and SOPs.
- Support continuous improvement efforts by contributing to workflow optimization, quality initiatives, and care process enhancements.
- Active and unencumbered Registered Nurse (RN) license, with compact licensure required and additional state licenses preferred.
- Minimum of 4 years of nursing experience across hospital, ambulatory care, primary care, case management, or health plan environments.
- Experience in triage, intake, care coordination, case management, or population health strongly preferred.
- Bachelor of Science in Nursing (BSN) required; Master of Science in Nursing (MSN) a plus.
- Strong clinical assessment skills with experience using protocols, decision trees, and structured care frameworks.
- Proficiency in motivational interviewing and patient engagement techniques to support behavior change and adherence.
- Comfort working with digital health tools, virtual care platforms, and multiple electronic systems simultaneously.
- Excellent communication, organizational, and time management skills with the ability to manage competing priorities.
- Ability to thrive in a fast-paced, evolving environment with a high degree of autonomy and collaboration.
- Spanish fluency is preferred but not required.
- Annual salary range: $80,000 – $85,000 USD
