Fully accredited by The Joint Commission, we offer a full range of services including 24/7/365 Emergency Department and Level IV Trauma Center, hospitalist program, testing, surgery, orthopedics, rehabilitation, and more so that you can experience better health.   Job Summary and Responsibilities As a Hospice Home Health Aide, you are more than a caregiver – you are a cornerstone of comfort, providing essential support that allows our patients to live their final chapters with dignity in their own homes. Every day, you will provide essential, compassionate hands-on care, ensuring dignity and well-being for our patients. To be successful in this role, you will possess a dedicated spirit of service, strong communication skills, a meticulous approach to patient support, and an unwavering commitment to our mission of dignified, high-quality healthcare. Responsibilities:Personal Care & Comfort: Assisting with personal hygiene, mobility (ambulation and transfers), and promoting overall patient well-being.Patient Monitoring: Diligently reporting any changes in condition to the nursing team.Supportive Environment: Performing light housekeeping, preparing meals, and ensuring a safe, healthful, and comforting home environment. Job Requirements High School Diploma or equivalent required.Certified Nurse Assistant (CNA) in the state of Minnesota (MN) required. Basic Life Support (BLS) from the American Heart Assocaition (AHA) required. Must have a valid Driver’s License.
