In this role, you will deliver primary care and preventive services as part of a care team actively redesigning how we approach healthcare — focusing on the health outcomes of your panel, member experience, and waste reduction rather than traditional productivity metrics. This is an opportunity to practice medicine the way you envisioned it: with time for members, room for clinical creativity, and the support of collaborative colleagues.POSITION DUTIES & RESPONSIBILITIESDeliver high-quality, evidence-based primary and preventive care through in-person, virtual, and telephone visitsManage a panel of members with focus on population health and individual member outcomesCollaborate with interdisciplinary care team members to design and implement member-centered care protocolsParticipate in continuous quality improvement initiatives to enhance clinical outcomes and member experienceMaintain accurate clinical documentation and engage with data-driven insights to inform clinical decision-makingContribute to the development and refinement of clinical workflows and care processes within a team-based modelEngage in professional development and evidence-based practice discussions with team membersDocument and code all member contacts accurately and legibly in the electronic medical recordFunction as an active member of the medical team and participate in care team meetingsContribute to organizational initiatives including clinical committees, education, quality review, and other mission-aligned activitiesDESIRED PROFESSIONAL SKILLS & EXPERIENCERequired:Current Texas medical license or ability to obtain oneBoard certification in Family Medicine or Internal MedicineCurrent DEA and CPR certificationValid Texas Driver's LicenseAbility to meet and maintain current organizational credentialing and privileging requirementsStrong clinical judgment and excellent diagnostic skillsProven track record of delivering high-quality, patient-centered careExcellent communication and interpersonal skillsComputer literacy with ability to navigate electronic health record systemsAbility to interact effectively with persons from diverse backgroundsSound judgment, flexibility to shift priorities, and commitment to confidentialityPreferred:Experience with or interest in value-based care modelsFamiliarity with electronic health records and health data analyticsExperience collaborating in team-based care environmentsInterest in population health and preventive careBilingual in English/SpanishPhysical Requirements:Ability to work irregular hours in a moderate-stress environmentCapacity to handle exposure to blood, body fluids, and communicable diseasesAbility to push/pull heavy objects and lift up to 50 poundsFull range of body motion for patient care activitiesWHAT WE OFFERCompetitive salary and potential quarterly and annual value-based bonus opportunitiesGenerous PTO10 paid holidaysCME Allowance and time offMedical, Dental, and Vision InsuranceOccurrence-based malpractice401(k) Investment PlanCompany EquityAt Harbor Health, we’re transforming healthcare in Texas through collaboration and innovation.
