Medical Records Clerk (Full-Time)
SHIFT: 8 Hour Shifts
POSITION SUMMARY
Responsible for ensuring the confidentiality, integrity, accuracy, daily maintenance, release of information and storage of all data both in an electronic and hard copy format consistent with the administrative, ethical, legal, and regulatory requirements. Performs duties to comply with legal release of protected health information in compliance with requests for records in accordance with applicable hospital policies and procedures, federal and state laws and regulations. Must always safeguard and protect the patient’s right to privacy by ensuring that only authorized individuals have access to the patient’s medical information and that all releases of information are in compliance with the request, authorization, hospital policy and HIPAA regulations. Outcomes of work has high impact on federal and state regulatory status, compliance, grant funding, coding and billing, payors and reimbursement, and timely accessibility of information for quality patient care and therefore must maintain knowledge of Title 22 requirements, and the Medical Staff Bylaws as they relate to chart completion requirements.
ESSENTIAL DUTIES
- The Medical Record Clerk performs prepping, scanning, quality control, and patient deficiency analysis of all documents received by the BVCHD Medical Records Department to maintain the integrity of the complex electronic patient medical record.
- Processes documentation into the Electronic Health Record (EHR) and is responsible for demonstrating knowledge of HIPAA privacy and security regulations as evidenced by appropriate handling of Protected Health Information (PHI), promoting confidentiality and using discretion when handling patient and various hospital departments’ information.
- Responsible for processing all release of information (ROI) specifically medical record requests in a timely and efficient manner ensuring accuracy and providing customers with the highest quality product and customer service.
- Abides by the standards of American Health Information Management Association (AHIMA) Code of Ethic, BVCHD Code of Conduct and reports to the HIM Director any violations identified or concerns.
- Accountability for compliance with laws, regulations, and policies to demonstrate ethical behavior.
- All other duties or responsibilities as assigned.
QUALIFICATIONS
Minimum Education (or substitute experience) Required:
- BLS Certification within 3 months of hire
Education Preferred:
- High School Diploma/GED
- Associates or bachelor’s degree in Health Information Management or other healthcare field
Minimum Experience Required:
- N/A
Experience Preferred:
- 1-2 years of experience in an acute-care hospital or healthcare facility
- 1-2 years of experience in medical records
- Ability to read and comprehend simple healthcare terminology

- Must be able to maintain the highest level of confidentiality
- Must be self-motivated and able to plan work activities and set effective priorities that are aligned with department productivity standards and BVCHD operational and fiscal goals
- Strong time management skills
- Excellent oral and written communication skills

- Ability to understand and become knowledgeable of Release of Information standards, policies & procedures and HIPAA regulations and to complete work in compliance of these and other standards.
- Critical thinking skills and strong attention to detail is required
- Ability to effectively use computer software and technology as required by the facility including Microsoft Word and Excel
