Job Functions:
- Guides CHF patients through inpatient, discharge, and outpatient phases of care to ensure continuity and timely access to needed services.
- Serves as the central point of contact and liaison between patients, families, and members of the multidisciplinary HVI team.
- Provides education on CHF disease processes, medications, lifestyle modifications, symptom management, and follow-up care.
- Engages patients in shared decision making in alignment with American College of Cardiology (ACC) guidelines and supports adherence to Guideline Directed Medical Therapy (GDMT).
- Participates in clinical discussions, HVI conferences, and coordination activities that support integrated patient care.
- Builds and maintains effective working relationships with physicians, APPs, nurses, and other care providers.
- Assists with data collection for registries, performance monitoring, and quality improvement initiatives related to CHF outcomes and care processes.
- Contributes to efforts aimed at improving patient experience and clinical effectiveness within the HVI Service Line.
- Maintains UpToDate knowledge of CHF therapies, treatment guidelines, and navigation best practices.
- Demonstrates competency in care coordination, patient education, communication, and professionalism.
- Completes all required annual education and training.
- Represents McLeod Health and the HVI Service Line professionally in both internal and community settings.
- Participates in community outreach events as requested.
- Performs other duties as required.
Qualifications /Training:
- A minimum of 3 years of recent cardiovascular experience.
- Strong knowledge of heart failure pathophysiology, GDMT medications, and other evidence-based guidelines.
- Excellent communication, patient education, and care-coordination skills.
- Ability to work independently and collaboratively across multidisciplinary teams.
Licenses/Certifications/Registrations/Education:
- Current South Carolina (or compact state) licensure as RN (SCLLR) in good standing required.
- Basic Life Support (BLS) certification required.
- Advanced Certification as approved by VP of Cardiovascular Services within 3 years of hire
