In this role, you will deliver comprehensive, member-centered care on an as-needed basis while upholding our commitment to excellence in healthcare delivery. This position offers flexible scheduling — with availability for approximately 5 shifts per month — while maintaining high-quality clinical standards and collaborative care approaches. The Advanced Practice Provider delivers medical care within their scope of practice as delineated by the supervising physician, in alignment with our mission of transforming healthcare through our payvider model. POSITION DUTIES & RESPONSIBILITIESProvide comprehensive primary and episodic care services to members on a PRN basisConduct thorough physical examinations, health assessments, and medical historiesDiagnose conditions and develop evidence-based treatment plans in collaboration with supervising physiciansOrder, interpret, and evaluate laboratory tests, diagnostic procedures, and imaging studiesPrescribe medications and treatments within scope of practice and regulatory guidelinesPerform clinical procedures appropriate to scope of practice and provider expertiseCounsel members and families regarding treatment plans, medication management, and health promotionCollaborate with interdisciplinary team members to ensure continuity and quality of careParticipate in care coordination and referral processes when appropriateDocument all member encounters accurately and thoroughly in the electronic health recordMaintain compliance with all clinical protocols, safety standards, and organizational policiesSupport quality improvement initiatives and evidence-based practice standardsRespond to member health concerns and provide appropriate follow-up care as needed DESIRED PROFESSIONAL SKILLS & EXPERIENCERequired:Graduate of an accredited Nurse Practitioner or Physician Assistant programCurrent state licensure as a Nurse Practitioner or Physician AssistantBoard certification by the appropriate national certifying body (NCCPA or nursing certification board)Current DEA registration with prescriptive authorityCurrent BLS CertificationMinimum 2 years of clinical experience in primary care, urgent care, or a relevant specialty settingStrong clinical assessment and diagnostic skills with sound clinical judgmentExcellent verbal and written communication skillsAbility to work independently while maintaining collaborative relationshipsFlexibility to work various shifts and adapt to changing clinical needsPreferred:Experience in ambulatory care, urgent care, or emergency medicine settingsProficiency with electronic medical record systemsExperience with value-based care delivery modelsKnowledge of chronic disease management and preventive care protocolsBilingual in English/SpanishPhysical Requirements:PRN scheduling with flexibility for approximately 5 shifts per month; 8-hour shifts with potential for shorter shifts as neededAbility to work across multiple clinic sitesAbility to meet and maintain current organizational credentialing and privileging requirements WHAT WE OFFERCompetitive hourly compensationFlexible PRN scheduling that accommodates your availabilityOccurrence-based malpracticeCollaborative team environment with physician supervision and support available At Harbor Health, we’re transforming healthcare in Texas through collaboration and innovation.
